TEAM CAMP REGISTRATION

To register for Player’s Edge Volleyball Camp complete form and submit.  Submission of this form serves as authorization to attend camp.

To register your team you must complete the area below.  If more than one team from your school is attending (varsity, junior varsity, etc.) submit an application for each team.  Team registration must be accompanied by a $50.00 non-refundable deposit/ application fee per person which will apply toward the total cost of the camp by May 1, 2018. Balance is due by July 1, 2018.  Please make payment with ONE school/personal check/money order/cashier check payable to Player’s Edge Volleyball.  Return to: Player’s Edge Volleyball 1304 Wellington Hills.
